Map wont Update afer saveing, please help with fix

Got a problem here, when im working on a map, when i save it and shut radiant down it loads ok, but when i add more stuff to the map, it saves (i think) but dont show up in the map, it seems like Map wont Update afer saveing, If i try save as new name it comes up with a error, this is what i get if i try and save it as a new file..

please help with reply fix, im sure ive seen a topic and a reply here on this subject befor, but cant remeber where i saw it..
